Cancelling your Appointment

If you are unable to attend an appointment with one of the doctors or nurses, please telephone the surgery.

By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

Test Results

Results of all tests may be obtained by telephone or calling at reception.

Some tests take longer than others therefore the doctor may not authorise results until all are returned from the laboratory. Please telephone between 2.00 - 4.00pm when results will have been reported on by the doctor. Please leave at least 4 working days before telephoning for results.

Please telephone after 4.00pm for urine results.


once it has been established that the individual lives within the Practice Area, they are asked to provide the following documents:


2. Photographic ID (Passport, Driving Licence)

3. Proof of address (Utility Bill, Bank Statement)

4. Completed HS200 or HSCR1 (see below which applies)

Patients registering from one GP practice to another either within Northern Ireland or transferring from Great Britain.


To register with the practice please bring your medical card to reception. You will also need to complete the following form: HS200 which can be printed by clicking on the link above or calling at reception. Please note you will also need to provide photographic ID on registration.

If you wish to register you must complete the following form HSC/R1 and provide the relevant supporting documentation. You can print the form by clicking on the link above or you can call into reception to pick up a form.

All new patients will be given an appointment to see the Doctor and the Practice Nurse. Please complete the questionnaire below and bring it to your new patient medical appointment.

Practice Boundary 

New Practice Boundary from 1st March 2019

The new practice boundary will only affect new patients moving into the area and wishing to register or current patients who move address outside the boundary.

Our practice boundary is a 7 mile radius.
